Yet another C-style vtable replaced with C++.

Hide struct FilteredAudioOutput from the plugins, preparing for hiding
MPD's core internals.

Prepare to add an abstract class AudioOutput, to be implemented by
plugins, to get rid of the C-style vtable.

This eliminates some overhead, because the compiler doesn't need to
consider these functions throwing.

Eliminate dependency on io_thread_get().

The old struct config_param remains only for top-level string options.

Make it "const".

Replaces GLib's GError.

Unnecessary overhead.

Pass audio_output objects around instead of void pointers.  This will
give some more control to the plugin, and prepares for non-blocking
audio outputs.

Move the "extern" declarations from output_list.c, for more type
safety.

After we've been hit by Large File Support problems several times in
the past week (which only occur on 32 bit platforms, which I don't
have), this is yet another attempt to fix the issue.
