Inserted the *_S_* macros right before the corresponding *_E_* block
Reindented the values.
Removed the duplicate set of E_xxx macros since they are only used on
non WIN32 platforms.
Uncommented the CO_E_INIT_ set of macros.
Removed some duplicate macro definitions and fixed various typos.
Added the definitions needed by the MFC.
