It does not make sense to use D3DLOCK_DISCARD for locking a newly created vertex buffer because it will be allocated, freed, and then allocated again.

Add some output_xxx() functions so we can print strings that don't come from resources. Also protect against buffer overflows in output_vprintf() and constify the output_xxx() functions.
